If you register for an account through the RelayHealth website, you will still have to come into the hospital and present your valid photo ID and fill out an authorization form to have access to the patient portal.
Go to PortalMyCRMChealth.org, a patient portal powered by RelayHealth, is an internet-based software product used by health care providers nationwide. It makes communication safer and easier while reducing costs. All Cookeville Regional physicians are encouraged to use myCRMChealth.org to effectively and securely communicate with their patients online.
There is no charge for access or routine communications such as appointment requests, lab and imaging results.
The service simply requires an internet connection and a web browser such as Internet Explorer, Firefox, Google Chrome, etc.
RelayHealth maintains the strictest privacy and security measures to protect your personal health information. Only authorized parties can access your records. The service features built-in encryption technology, hardware and software security measures and a thorough privacy policy.

Yes, but the patient is required to sign a medical release form.
For patients at Cookeville Regional Medical Center: You will need to visit Cookeville Regional’s Medical Records office before you are able to access their medical records. You can sign this form by visiting the Cookeville Regional campus and entering in the main lobby. If you are unable to find the Medical Records office, please ask a Cookeville Regional representative, and they will be happy to show you where to go.
You must be 18 or older to have a myCRMChealth.org account. A minor can ask a parent or guardian to link to an account. Some exceptions for minors may apply.
Upon registering, you will be taken to the home page in your account. To see your results, you will need to add your provider.
Click the “PROVIDER” button at the top of the page. A new screen will appear, and you will need to click “ADD PROVIDER OR FACILITY.”
In the search box, type Cookeville Regional Medical Center. Click “SELECT” beside the provider name to add to your account.
*By adding a provider to your account, you will be able to see your results. Without connecting to a specific provider, your results will not be visible.

AthenaHealth takes the security and privacy of protected health information very seriously and features a thorough privacy policy. They have implemented technical, administrative and physical safeguards, which are designed to protect your information from unauthorized use and access.

Yes, but the patient is required to sign a patient portal authorization form.
Access to any family member’s account will be put on hold or declined without the authorization form completed. This form requires signatures from both the family member and the proxy requesting access. You can sign this form by visiting your physician’s office.

Once your provider has received and read your results, they will be released in your Patient Portal account.
