Education: Graduate of a school of practical Nursing. Current Tennessee LPN license
Hours: 8a-4:30p
Experience: Experience in home health nursing preferred
Skills: BLS required at hire or during orientation, IV Qualified. Must possess ability to work well under pressure, to function independently and dependently, and to be flexible in applying knowledge to a variety of situations. Must deal with organization, personnel and public demands. Must possess strong oral and written communication skills, diplomacy and tack; demonstrates personal traits of a high level of motivation, team orientation, professionalism, and trustworthiness. Places a high value on treating others with dignity and respect.
Description:

This position is responsible for receiving, documenting and processing referrals for the home health agencies. Must have a working knowledge of accepted insurance providers and approved coverage areas.  May also provide nursing care utilizing the nursing process outlined in home health policies and procedures. Observes, assesses, treats and cares for patients demonstrating an understanding of the cognitive, physical, emotional and chronological maturation process.  May assist with nursing duties managed within the home health office.

Education: Graduate of a school of Practical Nursing. Current Tennessee LPN license.
Hours: Varied
Experience: Experience in home health preferred or 1 year hospital experience.
Skills: BLS required at hire or during orientation, IV qualified. Valid TN Drivers License

This position is responsible for providing professional nursing care to patients of the agency they will assess, treat and care for patients demonstrating an understanding of the cognitive, physical, emotional and chronological maturation process; and coordinate care planning with other disciplines.  Oversees the work of other professional and non-professional personnel. This position will provide services to geriatric, adult, adolescent, pediatric, and infant patients.

Physical Therapist

Education: Graduate of a school of an approved curriculum of physical therapy, and licensed to practice Physical Therapy in the State of Tennessee.
Hours: 8a-4:30p
Experience: Two years experience with 1 year in home health preferred.
Skills: Previous experience preferred. BLS required at hire or during orientation. Valid Tennessee drivers license.
