Cookeville Regional Medical Center Patient Portal

With your Cookeville Regional Medical Center Patient Portal you can:

  • View your Personal Health Record (PHR) online
  • Record your own personal health records
  • Review your hospital discharge information
  • View your lab and imaging results*
  • Manage your dependents’ care
  • Manage your medication lists

*Please note: some lab and imaging results will not be available

 

Ways to sign up: 

  •  If you are a patient at Cookeville Regional Medical Center, give your email address to a staff member in Registration at CRMC  and you will receive an auto-invite to connect using our online patient portal. By following the link provided in the email, you can create your own free online patient portal account. 
  •  You can also sign up for our patient portal by visiting the Medical Records office at CRMC. You must bring a valid photo I.D. and complete a release form. You then will be sent an email inviting you to connect with our patient portal. 

 **If you register for an account through the Relay Health website, you will still have to come into the hospital and present your valid photo I.D. and fill out an authorization form to have access to the patient portal. **

What is myCRMChealth.org?

myCRMChealth.org, a patient portal powered by RelayHealth, is an Internet-based software product used by health care providers nationwide. It makes communication safer and easier, while reducing costs. All Cookeville Regional physicians are encouraged to use myCRMChealth.org to effectively and securely communicate with their patients online.

 

Is there a fee for using myCRMChealth.org?

There is no charge for access or routine communications such as appointment requests, lab and imaging results.

 

Do I need special computer hardware or software to use myCRMChealth.org?

The service simply requires an Internet connection and a web browser such as Internet Explorer, Firefox, Google Chrome, etc.

 

Is the service secure and private?

RelayHealth maintains the strictest privacy and security measures to protect your personal health information. Only authorized parties can access your records. The service features built-in encryption technology, hardware and software security measures and a thorough privacy policy.

 

How do I get started?

It’s easy.

Two ways to sign up: 

If you are a patient at Cookeville Regional Medical Center, give your email address to a staff member in Registration at CRMC  and you will receive an auto-invite to connect using our online patient portal. You can also sign up for our patient portal by visiting one of our Cookeville Regional Medical Group physician offices. You must bring a valid photo I.D. and complete a release form. You then will be sent an email inviting you to connect with our patient portal.

 

Can I manage other dependents on my account?

Yes, but the patient is required to sign a medical release form.

For patients at Cookeville Regional Medical Center: you will need to visit Cookeville Regional’s Medical Records office before you are able to access their medical records. You can sign this form by visiting the CRMC campus and entering in the main lobby. If you are unable to find the Medical Record’s office, please ask a CRMC representative and they will be happy to show you where to go. Or you can email the form to bnbush@mycrmchealth.org or faxed to (931) 526-8814. 

 

What is the age requirement to have a personal patient portal?

You must be 18 or older to have a myCRMChealth.org account. A minor can ask their parent or guardian to link to an account. Some exceptions for minors may apply.

 

What if I have registered and still cannot see my results?

 Upon registering you will be taken to the home page in your account. To see your results you will need to add your provider. Click the “PROVIDER” button at the top of the page. A new screen will appear and you will need to click “ADD PROVIDER OR FACILITY.” In the search box type Cookeville Regional Medical Center. Click “SELECT” beside the provider name to add to your account.
 
*By adding a provider to your account you will be able to see your results. Without connecting to a specific provider your results will not be visible.
 

When will I see my results?

Most lab results should be released within 36 hours, excluding HIV and pregnancy tests.  Most other results like imaging, HIV and pregnancy tests will be released within 96 hours.
*some lab and imaging results will not be available.
