Director Case Management/Social Services Apply

Cookeville Regional Medical Center is seeking a full-time Director of Case Management/Social Services.

With a central focus on census  management, patient care outcomes and key care indicators the Director oversees the interdisciplinary plan of care and the discharge planning process to ensure the effectiveness and appropriateness of services. They will be a patient and family advocate to ensure that services are delivered to meet the needs of patients and families and that the utilization of resources is appropriate.  The Director of Case Management is responsible for the oversight of the Utilization Review procedures as it relates to general discharge planning and specifically as it relates to meeting criteria and requirements for Medicare, Medicaid coverage and that of HMO or private insurers.   Collaborates with all disciplines including staff, payers, system partners, vendors and community agencies to promote and ensure that the proper quality of care is provided and outcomes are achieved in a timely and cost-effective manner. Utilizes an interdisciplinary process to assess, plan and provide patient care for specific diagnoses from pre-admission through aftercare. The Director assists in organizing internal processes and systems to prevent avoidable hospital days. They will promote appropriate post-hospital utilization of resources, adequate education of patients and hospital staff, and decreased incidence of re-admissions. The Director is responsible for overseeing the Social Services Department with functions including assisting patients and their families in addressing social, emotional and economic issues associated with illness or injury. Other duties in that area include crisis intervention, discharge planning, continuity of care, financial assistance and referrals to community resources.

Education: Bachelor of Science in Nursing preferred. Equivalent combination of education, training, and experience may substitute for education requirements.

Licensure: Most hold a current Tennessee Nursing license.

Experience: Three to five years’ experience as the Director or Manager of Case Management preferred.
